JOB TITLE: Registered Nurse

OVERVIEW:

The staff RN ensures the delivery of efficient and effective nursing care while achieving positive clinical outcomes and patient/family satisfaction. A licensed professional, the RN has accountability for a group of patients and collaborates with physicians and multidisciplinary team members to provide physical and psychological support to patients, friends, and families. The RN also supervises assigned unit team members.

JOB DESCRIPTION:

·Identifies patient care requirements by establishing personal rapport with patients and other persons in a position to understand care requirements.

·Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.

·Promotes patient’s independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; answering questions.

·Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.

Helps patients by supporting personal hygiene and daily living needs; providing comfort, transportation, and vital sign monitoring.

 JOB DESCRIPTION:

·         Assesses patient condition by monitoring and recording vital signs and weight; reviewing patient care journal and keeping staff informed.

·         Assists Residents with activities of daily living which may include meals, personal hygiene and adjunct care needs.

·         Records patient information by making entries in the patient journal; notifying agency nursing supervisor of changing or unusual conditions.

 RESPONSIBLE TO:

The RN MDS Coordinator facilitates the completion and submission of accurate and timely interdisciplinary MDS Assessments, CAAs and Care Plans according to CMS RAI Manual Regulations and in accordance with all applicable laws. Regulations and Life Care Standards.

EDUCATION, EXPERIENCE AND LICENSURE REQUIREMENTS

  • Associate or bachelor’s degree in nursing from an accredited college or university
  • Currently licensed/ registered in Washington State. Must maintain an active Registered Nurse (RN) license in good standing throughout employment.
  • Two (2) years nursing experience. Geriatric nursing experience preferred.
  • Two (2) years MDS Nurse Experience and demonstrated ability to complete the RAI process by utilizing an interdisciplinary approach.
  • CPR certification upon hire or obtain during orientation. CPR Certification must remain current during employment.
  • Requires computer skills to complete MDS in a timely fashion.

ESSENTIAL JOB FUNCTIONS

  • Coordinate and assist with completion and submission of interdisciplinary, accurate and timely MDS Assessments, CCAs and Care Plans according to CMS RAI Manual Regulations
  • Report any changes in patient’s condition identified by the MDS Assessment to the DNS
  • Provide education to direct care associates regarding updates or changes to the CMS RAI Manual or Skilled Nursing Facility Regulations that impact documentation.
  • Assist with review of the Interdisciplinary Comprehensive Care Plan
  • Review Final Validation Reports and attest that all assessments have been completed and accepted into the CMS QIES system prior to billing and notify the Business Office when assessments are not ready to bill.
  • Review CMS Reports to identify assessments completed or submitted late and develop systems and processes to prevent reoccurrence.
  • Attend and participate in the daily Clinical Meeting, weekly Medicare meeting and other meetings upon request.
  • Perform functions of a staff nurse as required.
  • Exhibit excellent customer service and a positive attitude towards patients.
  • Demonstrate dependable, regular attendance.
  • Concentrate and use reasoning skills and good judgement.
  • Communicate and function productively on an interdisciplinary team.
  • Read, write, speak, and understand the English language.

We are a Seattle based non-profit five star nursing home currently seeking an experienced Full Time Staff Development & Infection Control Nurse. Responsibilities include facility orientation and in-service education in accordance with current state, federal and local regulations. This position coordinates the development, implementation, and evaluation of the overall Quality Improvement and Infection Control Programs under the direction of the Director of Nursing Services.

Job Requirements & Qualifications:

· Current RN license

· Must be a graduate of an accredited nursing school with current RN license, BSN preferred
